Despite Eren wielding a small stick and striking a grown man in both timelines, Mikasa is not waiting around the corner to pull him out of danger. Instead, in this timeline, Eren is beaten up by the grown man he assaults, and the bullies who beat him and Armin are witnesses to the event. Does Mikasa remember this scene, judging by her expression? Is this why Mikasa has become overly protective of Eren since the Battle of Trost? She's done this a million times and always knows how it will end.
That's why, at the end of Chapter #138, when Mikasa chose to kill Eren, Ymir smiled. That decision liberated her in some way, but only Ymir knows why. When Eren and Armin finish their conversation in the Path, he erases Armin's memory of it, which Armin recovers in Chapter #139 after Eren dies. Did Mikasa murder Eren in Chapter 139?
Mikasa responded to a special someone, they ran away and lived the remaining 4 years peacefully, and on Eren's final day, we saw Chapter 1 "See you later, Eren," that reality ended and restarted.
Now, after the restart, we have the current AoT. (Chapter 1 - Eren had a long dream). Eren went over everything again, and now we're at the point where she asked Mikasa again and got a different answer, family...
This bond between them alerts her to the fact that Eren's head and spine are within the Titan's mouth, and with Armin's assistance, she breaks her way through to the true Eren. She kisses Eren goodbye as she separates his head from his spine (and thus his Titan transformation). In the fantasy realm, she kisses Eren as he falls asleep, but the final page of the chapter quickly reveals that she is kissing Eren's severed head. It's a complicated moment between Mikasa and Eren, given how much trouble he's caused her with this final arc overall; this is one of the messiest goodbyes the two of them could have imagined. Mikasa Dream Chapter 138
Of course, there is no confirmation, but it is possible that it is Eren's child...but I don't see why we need a child at all, let alone Eren's child, for the plot. To thwart the leader's plan, it made more sense for her to become pregnant (because it affects titan power/zacks fluid and she cannot consume to preserve zacks power or other titans power) and that too with a childhood friend who has nothing to do with royal lineage.
Mikasa's dream in Chapter 138 concludes with the same panel that begins Chapter 1 of AOT. Other than a time loop, I can't think of any other way this could happen. Eren awakens calmly in Chapter 1. If we continue with the time loop, it makes sense because he just died calmly to the Titan curse in the previous loop (138 dream). The manga represents the second time loop. Eren wakes up frantically in the anime. It is demonstrated to be the result of an incoherent dream, but what if we apply the time loop theory? The second time loop (the entire manga) ends Eren's life by decapitating him. In the anime (the third time loop), Child Eren wakes up frantically because he just saw Mikasa cut off his head through Adult Eren's eyes. Adult Eren knew why Mikasa was killing him, but Child Eren didn't.

All of the Eldians in the area of Fort Salta are instantly transformed into Titans. They careen off the cliff's edge and rush to the centipede. Reiner steels himself to prevent it from reaching Eren, despite the fact that it has summoned reinforcements. Armin is fighting Eren behind him, and the two titans are grappling. Reiner fights the centipede, and to his horror, he recognizes some of the Titans around him. As Falco dives into the fray, Pieck leaps off his back and transforms, biting on the centipede's head.
